Chondrodermatitis nodularis chronica helicis (CNCH) is a small, nodular, tender, chronic inflammatory lesion occurring on the helix of the ear, most often in men.{{cite book | vauthors = James W, Berger T, Elston D | date= 2005 | title = Andrews' Diseases of the Skin: Clinical Dermatology | edition = 10th | publisher = Saunders | isbn = 0-7216-2921-0}}{{rp|610}} it often presents as a benign painful erythematous nodule fixed to the cartilage of the helix or antihelix of the external ear.{{cite journal | vauthors = Salah H, Urso B, Khachemoune A | title = Review of the Etiopathogenesis and Management Options of Chondrodermatitis Nodularis Chronica Helicis | journal = Cureus | volume = 10 | issue = 3 | pages = e2367 | date = March 2018 | pmid = 29805936 | pmc = 5969795 | doi = 10.7759/cureus.2367 | doi-access = free }} Although the exact cause of the condition is unknown, it has been linked to head pressure on the ear while sleeping. Treatment options include the use of pressure-relieving devices or, in cases where that is not an option, surgery.{{cite journal | vauthors = Nielsen LJ, Olsen CH, Lock-Andersen J | title = Therapeutic Options of Chondrodermatitis Nodularis Helicis | journal = Plastic Surgery International | volume = 2016 | pages = 4340168 | date = 2016-01-27 | pmid = 26925262 | pmc = 4748103 | doi = 10.1155/2016/4340168 | doi-access = free }}

Causes

The cause of CNCH is unknown. Nonetheless, a number of variables are thought to be involved in the development of CNCH, with the onset being associated with microtrauma, prolonged high pressure, or dermal ischemia.{{cite journal | last1=Sehgal | first1=Virendra N. | last2=Singh | first2=Navjeevan | title=Chondrodermatitis nodularis | journal=American Journal of Otolaryngology | publisher=Elsevier BV | volume=30 | issue=5 | year=2009 | issn=0196-0709 | doi=10.1016/j.amjoto.2008.04.001 | pages=331–336| pmid=19720252 }}

= Risk factors =

Elastic tissue loss, vascular and connective tissue degradation, and skin and cartilage thinning are all brought on by aging.{{cite journal | last=Dreiman | first=Bernard B. | title=Chondrodermatitis Nodularis Chronica Helicis Treated With Antia-Buch Reconstruction: Review and Case Report | journal=Journal of Oral and Maxillofacial Surgery | publisher=Elsevier BV | volume=65 | issue=7 | year=2007 | issn=0278-2391 | doi=10.1016/j.joms.2005.10.044 | pages=1378–1382}} The onset of CNCH symptoms may be related to the cartilage's increased susceptibility to pressure damage as it ages and loses some of its flexibility. Additionally, older adults typically move less while they sleep, which puts even more pressure on the ear.

Epidemiology

Chondrodermatitis nodularis chronica helicis is relatively rare, but it prefers fair-skinned men between the ages of 40 and 80.{{cite journal | last1=Sanu | first1=A | last2=Koppana | first2=R | last3=Snow | first3=D G | title=Management of chondrodermatitis nodularis chronica helicis using a 'doughnut pillow' | journal=The Journal of Laryngology & Otology | publisher=Cambridge University Press (CUP) | volume=121 | issue=11 | date=May 24, 2007 | issn=0022-2151 | doi=10.1017/s0022215107008535 | pages=1096–1098| pmid=17524165 | s2cid=34025503 }}{{cite journal | last1=Yaneza | first1=M M C | last2=Sheikh | first2=S | title=Chondrodermatitis nodularis chronica helicis excision and reconstruction | journal=The Journal of Laryngology & Otology | publisher=Cambridge University Press (CUP) | volume=127 | issue=1 | date=December 7, 2012 | issn=0022-2151 | doi=10.1017/s0022215112002721 | pages=63–64| pmid=23217242 | s2cid=206237570 }} Despite the fact that women are affected by CNCH, the reported male to female ratio is 10 to 1. CNCH usually affects one side of the body, although reports of bilateral lesions have indicated an incidence of 3% to 7%.{{cite journal | last1=Oelzner | first1=Sophia | last2=Elsner | first2=Peter | title=Bilateral chondrodermatitis nodularis chronica helicis on the free border of the helix in a woman | journal=Journal of the American Academy of Dermatology | publisher=Elsevier BV | volume=49 | issue=4 | year=2003 | issn=0190-9622 | doi=10.1067/s0190-9622(03)00482-1 | pages=720–722| pmid=14512925 }} After developing CNCH on the contralateral ear, patients who switch their sleeping side typically develop bilateral lesions.{{cite journal | last1=de Ru | first1=J. A. | last2=Lohuis | first2=P. J. F. M. | last3=Saleh | first3=H. A. | last4=Vuyk | first4=H.
