Main ingredients

The active ingredient in Prilosec OTC is delayed release omeprazole which is in the drug class proton pump inhibitors or PPIs. The “proton pumps” are present in parietal cells in the stomach and responsible for stomach acid production. There are 3 pathways the body uses to tell these cells to produce more stomach acid. Prilosec OTC prevents excess stomach acid production by turning off these acid producing pumps, and blocking heartburn before it starts. The benefits of Prilosec OTC last for up to 24 hours while the stomach makes new acid pumps. While proton pump inhibitors like Prilosec OTC start working on the first day, it can take up to 4 days for some people to see the full benefits.{{Cite journal |last1=McRorie Jr. |first1=Johnson W. |last2=Gibb |first2=Roger D. |last3=Miner Jr. |first3=Philip B. |date=2014 |title=Evidence-based treatment of frequent heartburn: The benefits and limitations of over-the-counter medications |url=https://onlinelibrary.wiley.com/doi/abs/10.1002/2327-6924.12133 |journal=Journal of the American Association of Nurse Practitioners |language=en |volume=26 |issue=6 |pages=330–339 |doi=10.1002/2327-6924.12133 |pmid=24825071 |issn=2327-6924}} Other common treatment options for heartburn are antacids and histamine receptor agonists (H2RA’s).  Antacids are products that show up on the pH scale as basic and work to neutralize the stomach acid through an acid-base reaction, their effects can last for up to a few hours.  H2RA’s work by blocking the histamine receptor on the cells that produce stomach acid, so that the body can’t use that pathway to increase stomach acid.{{Cite web |title=Take the "Burn" out of Heartburn |url=https://pharmacist.therapeuticresearch.com/Content/Segments/PRL/2016/Aug/Take-the-Burn-out-of-Heartburn-10028 |access-date=2025-05-16 |website=pharmacist.therapeuticresearch.com}} {{Cite journal |last1=McRorie Jr. |first1=Johnson W. |last2=Gibb |first2=Roger D. |last3=Miner Jr. |first3=Philip B. |date=2014 |title=Evidence-based treatment of frequent heartburn: The benefits and limitations of over-the-counter medications |url=https://onlinelibrary.wiley.com/doi/abs/10.1002/2327-6924.12133 |journal=Journal of the American Association of Nurse Practitioners |language=en |volume=26 |issue=6 |pages=330–339 |doi=10.1002/2327-6924.12133 |pmid=24825071 |issn=2327-6924}}However, the H2RA’s do not affect the other 2 pathways the body has for turning on the acid pumps.   Over the counter antacids can be used multiple times throughout the day as needed to treat heartburn while the H2RA’s can be used up to twice daily.{{Cite journal |last1=McRorie Jr. |first1=Johnson W. |last2=Gibb |first2=Roger D. |last3=Miner Jr. |first3=Philip B. |date=2014 |title=Evidence-based treatment of frequent heartburn: The benefits and limitations of over-the-counter medications |url=https://onlinelibrary.wiley.com/doi/abs/10.1002/2327-6924.12133 |journal=Journal of the American Association of Nurse Practitioners |language=en |volume=26 |issue=6 |pages=330–339 |doi=10.1002/2327-6924.12133 |pmid=24825071 |issn=2327-6924}}
