Botryoid odontogenic cyst (BOC) is a type of developmental odontogenic cyst that is extremely rare.{{cite journal | last1=Arora | first1=Piyush | last2=Bishen | first2=KundenduA | last3=Gupta | first3=Nishant | last4=Jamdade | first4=Anshuman | last5=Kumar | first5=GopaR | title=Botryoid odontogenic cyst developing from lateral periodontal cyst: A rare case and review on pathogenesis | journal=Contemporary Clinical Dentistry | publisher=Medknow | volume=3 | issue=3 | year=2012 | pages=326–329 | issn=0976-237X | doi=10.4103/0976-237x.103629 | doi-access=free | pmid=23293492 | pmc=3532799 }} It is thought to be a lateral periodontal cyst (LPC) variant with a higher risk of recurrence.{{cite journal |last1=Méndez |first1=Pedro |last2=Junquera |first2=Luis |last3=Gallego |first3=Lorena |last4=Baladrón |first4=Jaime |title=Botryoid odontogenic cyst: clinical and pathological analysis in relation to recurrence |journal=Medicina oral, patología oral y cirugía bucal |date=December 1, 2007 |volume=12 |issue=8 |pages=E594–E598 |pmid=18059246 |url=http://www.medicinaoral.com/pubmed/medoralv12_i8_pE594.pdf |access-date=17 November 2023 |publisher=Valencia, España : Medicina Oral S.L. |location=Spain |issn=1698-6946}} Weathers and Waldron coined the term BOC in 1973.{{cite journal | last1=Weathers | first1=Dwight R. | last2=Waldron | first2=Charles A. | title=Unusual multilocular cysts of the jaws (botryoid odontogenic cysts) | journal=Oral Surgery, Oral Medicine, Oral Pathology | publisher=Elsevier BV | volume=36 | issue=2 | year=1973 | issn=0030-4220 | doi=10.1016/0030-4220(73)90244-2 | pages=235–241| pmid=4515756 }} Adults over the age of 50 are the most affected.{{cite journal | last1=Üçok | first1=Ö. | last2=Yaman | first2=Z. | last3=Günhan | first3=Ö. | last4=Üçok | first4=C. | last5=Doğan | first5=N. | last6=Baykul | first6=T. | title=Botryoid odontogenic cyst: report of a case with extensive epithelial proliferation | journal=International Journal of Oral and Maxillofacial Surgery | publisher=Elsevier BV | volume=34 | issue=6 | year=2005 | issn=0901-5027 | doi=10.1016/j.ijom.2005.01.005 | pages=693–695| pmid=16053898 }} BOC appears as a slow-growing lesion that is symptomatic in approximately 70% of cases.{{cite journal | last1=de Andrade Santos | first1=Pedro Paulo | last2=Freitas | first2=Valéria Souza | last3=de Almeida Freitas | first3=Roseana | last4=Pinto | first4=Leão Pereira | last5=de Souza | first5=Lélia Batista | title=Botryoid odontogenic cyst: A clinicopathologic study of 10 cases | journal=Annals of Diagnostic Pathology | publisher=Elsevier BV | volume=15 | issue=4 | year=2011 | issn=1092-9134 | doi=10.1016/j.anndiagpath.2010.03.008 | pages=221–224| pmid=20952285 }}
Signs and symptoms
BOC patients frequently complain of swelling, as well as pain and paraesthesia. BOCs are larger cysts that range in size from 4 mm to 45 mm and can be unilocular or multilocular.
Diagnosis
The BOC has a thin epithelial cyst lining composed of flattened squamous or cuboidal cells with focal plaque-like thickenings under the microscope. Clear cells may be seen in the cyst lining or among the epithelial rests dispersed among the fibrous connective-tissue wall on rare occasions.{{cite journal | last1=Yildirim | first1=Benay | last2=Baris | first2=Emre | title=Botryoid Odontogenic Cyst with Extensive Clear Cells | journal=Biomedical Journal of Scientific & Technical Research | publisher=Biomedical Research Network+, LLC | volume=19 | issue=5 | date=July 19, 2019 | pages=14589–14591 |url=https://biomedres.us/fulltexts/BJSTR.MS.ID.003360.php | access-date=November 18, 2023}}
In terms of appearance, BOC shares similarities with some odontogenic tumors such as ameloblastoma, odontogenic myxoma, adenomatoid odontogenic tumor, and others. Incisional biopsy can be used to perform preoperative differential diagnosis.
