It is a left tributary of the River Thame. It rises in a number of natural springs at the foot of the Chiltern Hills near Watlington, and flows in a northwesterly direction for approximately {{convert|7|mi}} through Cuxham, Chalgrove, and Stadhampton, before emptying into the Thame near Chiselhampton.{{cite web |title=Watlington Chalk Stream |url=https://www.wildoxfordshire.org.uk/case-studies/watlington-chalk-stream |publisher=Wild Oxfordshire |access-date=1 June 2024}}

The brook floods occasionally, especially in and downstream of Chalgrove. To relieve this, a man-made conduit was built in Chalgrove, splitting the stream in two.
