Jan Polák (born 26 March 1989) is a Czech former professional footballer who played as a centre-back.

Playing career

Polak joined Polish first division club Piast Gliwice in August 2012{{cite web|title=Czeski obrońca wzmocni Piasta?|url=http://www.piast.gliwice.pl/site/?tag=jan-polak|publisher=piast.gliwice.pl|accessdate=20 August 2012|language=pl}} and played with them for two seasons.
