John Leo Falconbridge Cook (December 4, 1892 – September 3, 1964[http://www.sihrhockey.org/__a/members/deaths_by_year.cfm?year=1964&month=9 Hockey deaths - September 1964]) was a Canadian professional ice hockey player.
