It is a structure built out of coral limestone and concrete, with an interior chamber about {{convert|3|x|1.8|m|ft}} in size. The entrance is on the south side, measuring {{convert|0.85|x|1.45|m|ft}}. There is one gun port, facing north, measuring {{convert|0.32|x|.9|m|ft}}. The structure was designed to be well hidden in view from the sea and air.{{cite web|url={{NRHP url|id=88001886}}|title=NRHP nomination for Mana Pillbox|publisher=National Park Service|accessdate=2015-06-04}} It was built, probably by conscripted Chamorro labor, during the Japanese occupation period 1941–44.

The structure was listed on the National Register of Historic Places in 1991.
