The Reconstruction Agency established three Regional Offices for Reconstruction in Iwate, Miyagi, and Fukushima prefectures, and two smaller regional offices in Hachinohe, Aomori Prefecture and Mito, Ibaraki Prefecture. The agency also sought to work with business associations in Japan in order to establish or revive economic activity in the Tohoku region affected by the earthquake, tsunami, and nuclear disaster. The agency, as well as the Japan Business Federation, the Japan Association of Corporate Executives, the Japan Chamber of Commerce and Industry, and their affiliated corporations, formed the Reconstruction Design Council in response to the Great East Japan earthquake.{{cite book | last = Reconstruction Agency | script-title=ja:復興の現状と取組 | url = http://www.reconstruction.go.jp/topics/120521genjototorikumi.pdf | access-date = 2012-06-01 | date = 2012 | publisher = Reconstruction Agency | location = Tokyo | language = ja |trans-title=Conditions of reconstruction and planning}}

The establishment of the Reconstruction Agency received criticism for both the slow pace of its establishment, and for the location of its headquarters. Residents and officials in regions affected by the disaster, notably Yūhei Satō, governor of Fukushima Prefecture, publicly noted the lack of speed in which the agency was created:{{cite web | url = http://www.cnn.com/2012/02/26/world/asia/rebuilding-japan-overview/index.html | title = Japan one year on: What's changed? | author = Peter Shadbolt | date = 2012-03-01 | publisher = CNN | access-date = 2012-06-03}}
{{Quote|text=From the victims' perspective, I can't help but ask, 'Couldn't they have launched the agency more quickly?'|sign=Yūhei Satō|source=CNN, Mar. 1, 2012}}
Legislation in the National Diet to establish the agency was slowed by the resignation of Prime Minister Naoto Kan in September 2011. The bill to create the agency passed in December 2011, nine months after the disaster occurred, delaying the opening of the agency until February 2012. Kan acknowledged the slow pace of government response to the disaster on March 3, 2011, and pledged to speed up recovery efforts.{{cite news | title = Japan's Prime Minister Vows To Speed Up Reconstruction | first = Phred | last = Dvorak | url = https://online.wsj.com/article/BT-CO-20120311-705154.html | newspaper = Wall Street Journal | publisher = Dow Jones & Company, Inc. | location = New York, N.Y. | issn = 0099-9660 | oclc = 43638285 | date = 2012-03-11 | access-date = 2012-06-04}} Residents displaced by the tsunami have opposed Agency-led plans to rebuild towns on higher land away from the coast, and see it as a disconnect between the central government and the population in the Tōhoku region.{{cite news | title = Year after tsunami, battles rage in Japan over how, where to rebuild | url = http://www.miamiherald.com/2012/03/10/2685252/reconstruction-plans-stir-debate.html | newspaper = Miami Herald | publisher = Herald Print. and Pub.
