Add new section: "Failure in Design"

I'd like to add a section to this page regarding "Failure in Design" as I think it would enrich the page.

::{{Ping|Jnychka}} I don't think "access" needs to be granted? I don't think the article is protected in any way. As far as I know even people who did not create an account, editing anonymously/ though their IP addresses can edit the page. Wikipedia is the "encyclopedia that anyone can edit". I just want to alert you that additions should be covered by terciary or secondary sources, and that Original Research (WP:OR) is not welcome. Other than that, thanks for your interest in editing!
