Background

The song originally had a different instrumental; Three 6 Mafia member DJ Paul stated that the original had "keys and scary music in it", but remixed the beat one night.{{Cite news|url=http://www.thefader.com/2018/04/10/dj-paul-who-run-it-challenge-three-6-mafia-interview|title=DJ Paul explains the story behind Three 6 Mafia's "Who Run It"|work=The FADER|access-date=2018-05-22}} He played the new version on the group's tour bus to an overwhelmingly positive reception. The track on Three 6 Mafia Presents: Hypnotize Camp Posse has more instrumental after the final verse, which makes it slightly longer than the one on When the Smoke Clears: Sixty 6, Sixty 1.

Music video

The music video for the single features the group rapping in a Cadillac Escalade and driving around Memphis, as each member is introduced along with their respective verse. Juicy J states that the group was "high as hell" during filming, and feared that he would fall off of the car.{{Cite news|url=http://www.mtv.com/news/3073007/juicy-j-who-run-it-interview/|archive-url=https://web.archive.org/web/20180417192421/http://www.mtv.com/news/3073007/juicy-j-who-run-it-interview/|url-status=dead|archive-date=April 17, 2018|title=Juicy J On The Rebirth Of 'Who Run It': 'Three 6 Mafia's Music Will Never Die'|work=MTV News|access-date=2018-05-22}}
