The extreme capsule (Latin: capsula extrema) is a series of nerve tracts between the claustrum and the insular cortex.{{cite web |last1=Jones |first1=Jeremy |title=Extreme capsule {{!}} Radiology Reference Article {{!}} Radiopaedia.org |url=https://radiopaedia.org/articles/extreme-capsule |website=Radiopaedia}} It is also described as a thin capsule of white matter as association fibres.{{cite book |last1=Carpenter |first1=M |title=Core text of neuroanatomy |date=1985 |publisher=Williams & Wilkins |isbn=0683014552 |page=32 |edition=3rd}} The extreme capsule is separated from the external capsule by the claustrum, and the extreme capsule separates the claustrum from the insular cortex, and all these lie lateral to the corpus striatum components.{{cite book |last1=Carpenter |first1=M |title=Core text of neuroanatomy |date=1985 |publisher=Williams & Wilkins |isbn=0683014552 |page=252 |edition=3rd}}
From the midline of the brain to the side, the extreme capsule is the outermost from the external capsule and the inner internal capsule.{{cite book |last1=Haines |first1=D |title=Neuroanatomy : an atlas of structures, sections, and systems |date=2008 |publisher=Wolters Kluwer Health/Lippincott Williams & Wilkins |isbn=9780781763288 |page=146 |edition=7th}}
It is most easily visible in a horizontal section, just lateral to the claustrum.
