The Global Digital Exemplar (GDE) programme is an NHS England initiative to achieve digital transformation in selected exemplar organisations and to create a knowledge sharing ecosystem to spread learning from these exemplars.{{cite journal|last1=Cresswell|first1=Kathrin|last2=Sheikh|first2=Aziz|last3=Krasuska|first3=Marta|last4=Heeney|first4=Catherine|last5=Franklin|first5=Bryony Dean|last6=Lane|first6=Wendy|last7=Mozaffar|first7=Hajar|last8=Mason|first8=Kathy|last9=Eason|first9=Sally|last10=Hinder|first10=Susan|last11=Potts|first11=Henry W W|last12=Williams|first12=Robin|title=Reconceptualising the digital maturity of health systems|journal=The Lancet Digital Health|volume=1|issue=5|year=2019|pages=e200–e201|issn=2589-7500|doi=10.1016/S2589-7500(19)30083-4|pmid=33323267|doi-access=free|hdl=20.500.11820/61d2fa05-db65-4420-af50-f98b24330a2f|hdl-access=free}} The programme is to enable "digitally advanced"{{cite web |title=NHS England » Global Digital Exemplars |url=https://www.england.nhs.uk/digitaltechnology/info-revolution/exemplars/ |website=www.england.nhs.uk |accessdate=8 July 2018 |date=2 May 2018 |archive-date=9 July 2018 |archive-url=https://web.archive.org/web/20180709010053/https://www.england.nhs.uk/digitaltechnology/info-revolution/exemplars/ |url-status=live }} NHS trusts to share knowledge with other NHS trusts, specifically knowledge gained during the implementation of IT systems, and especially experience from introducing electronic health record (EHR) systems. Four rounds of exemplars have been announced so far — two waves of acute trust GDEs, and one wave each of ambulance trusts, and mental health trusts. In addition, eighteen acute trust "fast followers" have been partnered with the acute trusts.{{cite news |title=Eighteen NHS trusts confirmed as 'fast followers' to digital exemplars |journal=British Journal of Healthcare Computing |date=11 September 2017 |url=https://www.bj-hc.co.uk/eighteen-nhs-trusts-confirmed-fast-followers-digital-exemplars |accessdate=8 July 2018 |archive-date=9 July 2018 |archive-url=https://web.archive.org/web/20180709010232/https://www.bj-hc.co.uk/eighteen-nhs-trusts-confirmed-fast-followers-digital-exemplars |url-status=dead }}
The programme involves the investment of £395 million.{{Cite journal|doi = 10.1136/bmjopen-2020-041275|title = Formative independent evaluation of a digital change programme in the English National Health Service: Study protocol for a longitudinal qualitative study|year = 2020|last1 = Cresswell|first1 = Kathrin|last2 = Sheikh|first2 = Aziz|last3 = Dean Franklin|first3 = Bryony|last4 = Krasuska|first4 = Marta|last5 = Nguyen|first5 = Hung|last6 = Hinder|first6 = Susan|last7 = Lane|first7 = Wendy|last8 = Mozaffar|first8 = Hajar|last9 = Mason|first9 = Kathy|last10 = Eason|first10 = Sally|last11 = Potts|first11 = Henry|last12 = Williams|first12 = Robin|journal = BMJ Open|volume = 10|issue = 10|pages = e041275|pmid = 33033100|pmc = 7545630}} Each GDE will receive "up to £10 million" to spend on digital projects.{{cite news |last1=Heather |first1=Ben |title=Flagship digital fund finally reaches 'exemplar' trusts |journal=Health Service Journal |date=3 July 2017 |url=https://www.hsj.co.uk/technology-and-innovation/flagship-digital-fund-finally-reaches-exemplar-trusts/7019136.article |accessdate=8 July 2018 |language=en |archive-date=8 July 2018 |archive-url=https://web.archive.org/web/20180708221035/https://www.hsj.co.uk/technology-and-innovation/flagship-digital-fund-finally-reaches-exemplar-trusts/7019136.article |url-status=live }} The funding must be matched locally, but not necessarily in cash.
Programme elements
Each Global Digital Exemplar (GDE) received £10 million and their matched Fast Followers (FFs) received £5 million (£5 million for GDEs and £3 million for FFs in mental health); and they were required to secure matched funding internally. The Healthcare Information and Management Systems Society (HIMSS) Electronic Medical Record Adoption Model (EMRAM) was chosen as a guide for programme outputs, with GDEs expect to obtain HIMSS Level 7 and FFs, HIMSS Level 5.Independent Evaluation of the Global Digital Exemplar (GDE) Programme: 30 month report, https://www.ed.ac.uk/files/atoms/files/30_month_gde_evaluation_report_0.pdf {{Webarchive|url=https://web.archive.org/web/20201201141102/https://www.ed.ac.uk/files/atoms/files/30_month_gde_evaluation_report_0.pdf |date=1 December 2020 }}
The partnerships between GDEs and FFs constitute a formal mechanism to support knowledge transfer. The programme also introduced the idea of "Blueprints", documents describing how to implement digital technologies in healthcare.{{cite journal|last1=Williams|first1=Robin|last2=Sheikh|first2=Aziz|last3=Franklin|first3=Bryony Dean|last4=Krasuska|first4=Marta|last5=Nguyen|first5=Hung The|last6=Hinder|first6=Susan|last7=Lane|first7=Wendy|last8=Mozaffar|first8=Hajar|last9=Mason|first9=Kathy|last10=Eason|first10=Sally|last11=Potts|first11=Henry W W|last12=Cresswell|first12=Kathrin|title=Using Blueprints to promote interorganizational knowledge transfer in digital health initiatives—a qualitative exploration of a national change program in English hospitals|journal=Journal of the American Medical Informatics Association|year=2021|volume=28|issue=7|pages=1431–1439|issn=1067-5027|doi=10.1093/jamia/ocab020|pmc=8480174 |pmid=33706378|doi-access=free}}
