I find this template troubling for several reasons. The wording seems arbitrary:

Controversial edit? What the heck is that? One might think that perhaps it is related to our contentious topics procedures—but no. There doesn't seem to be any clear guidance about when it applies, or when it doesn't.

- Revert to original wording. This has been a useful template in the past, and could be again if the wording was less peculiar. - : I believe that every user warning template must be related to a possible violation of a specific guideline or policy and have a link to it, or else there is no point having the template, as the potential for abuse is too high and the user receiving it has the right to know what he is being accused of and how to avoid doing it again. So, what P&G is this about? Apparently at the outset, per Grutness's link, it was about vandalism, but we already have multi-level template {{tl|uw-vandalism1}} – {{tl|uw-vandalism4}} for that, so I am opposed to reverting to that language and having this one as a duplicate, as it would never be clear when to use uw-controversial, and when to use uw-vandalismN. (I am not opposed to "soft" versions of templates with gentler wording, but we even have that already: the {{tl|Uw-subtle1}}{{snd}}{{tl|Uw-subtle4}} series.)
- : I am very strongly opposed to making this template about Contentious topics. The wording of contentious topics has been worked out by the Arbitration Committee, applies specifically to three types of templates, along with a specific list of contentious topics, and is linked to arbitration enforcement. Tfd is very clearly not the venue where we are going to insert a new template into that process. (I don't believe we should even so much as redirect it to one of the ArbCom templates without their green light.)
- : So, if it is not about vandalism, and it is not about contentious topics, then what is it about, and what guideline/policy do we link to to make sure the user understands what the violation was?
